If your dentist is registered with us, you can apply for LAMP Dental Implant After Care through us directly.
Please download a
Patient Application Form
and send to:
LAMP Services Limited,
Chester House,
Harlands Road,
Haywards Heath,
West Sussex
RH16 1LR.
The annual premium is as follows:
- £75.00 per implant for new implants placed within the last year
- £60.00 per implant for existing implants placed over a year ago (if there has been no previous cover or a break in cover has occurred,
there will be a 6 month waiting period from the date we receive the application for these implant(s) for failure to integrate)
We accept payment by:
- Direct Debit
- Debit/Credit Card (except Electron, Solo & Amex)
- Cheque (made payable to LAMP Insurance Company Limited)
Once we receive your completed application form you will be issued with your Certificate of Insurance and Policy Documentation.
If your dentist is not registered with us, please direct them to this website for details on how they can join the scheme.
Alternatively, please let us know their details and we will contact them directly.
