You can apply for LAMP Dental Implant After Care through your dentist or through us directly. Please download an Application Form and send to:
LAMP Services Limited,

Haywards Heath,
West Sussex
RH16 1LR.
The annual premium is as follows:
- £75.00 per implant for new implants placed within the last year
- £60.00 per implant for existing implants placed over a year ago
We accept payment by:
- Direct Debit
- Debit/Credit Card (except Electron, Solo & Amex)
- Cheque (made payable to LAMP Insurance Company Limited)
Once we receive your completed application form you will be issued directly with your Certificate of Insurance and Policy Documentation.
